Community Health Center of Yavapai celebrates multiple Quality Improvement Awards

Community Health Center of Yavapai (CHCY) has been recognized by the U.S Department of Health and Human Services (HHS) for its commitment to providing high-quality care to the residents of Yavapai County. CHCY is proud to accept the Clinical Quality Improvers and Advancing Health Information Technology (HIT) for Quality awards, earning a total of $34,268 in grant funding.

These awards recognize CHCY’s use of Health Information Technology to increase access to care and advance quality of care for Yavapai County residents and CHCY’s significant quality improvement gains from the previous year.

CHCY’s innovative uses of HIT include the ability for patients to check in for their appointments and complete paperwork online, sending medication prescriptions electronically to pharmacies, and sharing information with hospitals and medical specialists through a health information exchange.  CHCY also has a patient portal which patients can use to access their medical chart, communicate with their medical provider, and schedule appointments.  CHCY actively monitors patient health information in order to improve its services.

CHCY’s quality improvement gains from 2018 to 2019 included getting more expectant mothers in for prenatal care during the first trimester of their pregnancy, an increased cervical cancer screening rate, an increased rate of screening and follow-up care for depression, and an increased number of children receiving dental sealants.

CHCY Director Sharon Rickman said, “While we are grateful for the funding we’ve received from these awards, I’m most grateful for CHCY’s commitment to quality, patient-centered care. From our providers, to front and back office staff, everyone works day in and day out to ensure all Yavapai County residents get the quality care they deserve. I’m especially thankful for how our team continues to pull together despite these uncertain and challenging times.”   

The grant funding received with these awards will be used to further improve the quality of care that CHCY offers to the people of Yavapai County.
