Community Health Center of Yavapai’s Patient Portal offers patients easy and private access to their own medical information on their personal computer or mobile device, so patients can view their own health record whenever and wherever they have access to the internet.

If you have already set up your NextMD account, login using the button below. Otherwise, please obtain a token number at your next appointment or call us at 928-583-1000. Once you have received a token number, begin registering using the button below.

The NextMD patient portal is a free service provided to you. With the Patient Portal, you can:
- Communicate with our staff and providers through a convenient, safe, and secure environment.
- View your medical records
- Request medication refills online
- Receive your statement online
- Request that we update your address or phone number
- Pay your bill online
- Receive lab results
Is my information safe?
Your security is important. We use technology to encrypt, safeguard, and secure your personal information. NextMD, is a protected web portal which allows secure access to us and to your health information.
I have forgotten my username. How can I retrieve it?
Go to nextmd.com
and click “Forgot Username?”. Enter your email address and your username will be emailed to you.
I have forgotten my password. How can I retrieve it?
Go to nextmd.com
and click “Forgot Password?”. You will be prompted to enter in either your username or email address associated with the account. You will then be asked to verify your account using the previously set up security questions and prompted to reset your password.
I have a new e-mail address. How can I change it?
Login to your NextMD account
and click on the “My Account Tab”. Go to “My Information” and then scroll down to email addresses, where you can update your email address.
